Our Adults' Case Management system is designed to be used by social workers, professionals in partner agencies such as health providers, the third sector and adults with care needs, as well as their carers or representatives.
The system provides flexible and comprehensive functionality to enable the management of contacts, referrals, assessment, and the setting of personal budgets, reablement plans, the planning of care and support, and commissioning of services for funded adults and self-funders.
It also provides safeguarding, Organisational Safeguarding, DOLS, Shared Lives Carer functionality, provider management, financial management and financial assessment; all within a logical and easy to navigate workflow, which can be tailored to reflect local authorities’ working practices.
There are integrated solutions for citizen access and interaction with their case (or access on their behalf by carers or representatives), and portals for providing information and signposting.
The seamless integration that the Adults' Case Management system shares with the embedded finance module, ContrOCC, puts financial information and control at the heart of the care management process.
Most customers using our Adults' Case Management system also use our Children’s Case Management system. The two systems can operate independently, or as part of an integrated whole. Our solutions allow data to be re-used as appropriate to save practitioner time and protect the individual.
