Our unique status as a supplier of software solutions across both the health and social care sectors provides obvious advantages when it comes to cross-sector collaboration. By integrating our EPR with social care systems, admission and discharge notices can be sent direct into the social care workflow in real time. This helps ensure that discharge processes are managed smoothly and efficiently and that individuals are well-supported as they move between hospital and community-based care.
Our EPR supports a ‘Unified View’ functionality which allows patient data from other NHS Trusts using the CareFlow EPR to be seen natively within a single, integrated clinician’s patient record view. In addition to highlighting the provenance of each data item, the Unified View also is able to support a read-only web page view of the acute record from one or more CareFlow EPRs, into third party systems such as Primary Care or Shared Care Record systems.
Our clinical collaboration tool, CareFlow Connect, supports cross-organisation, patient identified conversations between care teams by enabling organisations to form an ‘affiliated network’ and allowing their care teams to opt-in to being messaged by other care teams in that network. The conversations immediately become attached to the patient records of all the organisations involved in that conversation. This is being used today to support care co-ordination across Acute, Primary, Community and Social Care and the management and handover of patients in Virtual Wards.
As a centrally hosted solution, BadgerNet Maternity links with all BadgerNet Maternity and Neonatal sites across the UK. This means that units can be assured of having critical information where and when it is needed. With a detailed governance model in place, units can also opt in to joining a single pregnancy record in their region. This allows units to write to the same pregnancy record, ensuring that information is on hand when needed.
Medicines can be prescribed and administered in various clinical settings whether inpatient or outpatient. The solution creates a single record of prescription, administration and dispensing history which can be shared across different instances using a shared medicines formulary. The solution supports the recording of on-admission medicines which are used as the basis of the inpatient treatment, and on discharge, the system automatically reports any changes to these medicines as part of the medicines reconciliation workflow.
CareFlow’s interoperability features can also be used to contribute to shared care records and population health solutions.
